If you have spent any real time in the Lands Between, you know that the NPCs in Elden Ring are rarely just there for flavor. They usually want something from you, and in the case of Millicent, following her story from the rot-infested swamps of Caelid to the heights of the Haligtree is one of the most emotional and mechanically rewarding journeys in the game. I found that her quest is basically mandatory if you want to optimize a fast-attacking Dexterity build or if you accidentally got hugged by the Three Fingers and need a way out of the Frenzied Flame ending.
How to Start the Quest in Caelid
To get things moving, you need to head to eastern Caelid. I always look for a small wooden building called Gowry’s Shack, which is sitting right on the road leading into Sellia, Town of Sorcery. Inside, you will find Sage Gowry. He is an old man who seems desperate to save a girl named Millicent who is dying of scarlet rot in a church nearby.
He will ask you to find an Unalloyed Gold Needle, which is being held by a boss in the nearby swamp. One thing I noticed after the recent 1.12 update is that tracking items like this is much easier now. When you finally pick it up, it will show up in your recent items tab with a little exclamation mark, so you do not have to go digging through a massive inventory to make sure you actually got it.
Beating Commander O’Neil and the Unalloyed Gold Needle
The needle you need is guarded by Commander O’Neil. You can find him in the heart of the Swamp of Aeonia, surrounded by a bunch of spectral soldiers. I have fought this guy on multiple characters, and the best tip I can give you is to use the environment. There are scarlet rot geysers all over the arena that deal massive damage to him if you kite him over them.
Once he is down, he drops the Unalloyed Gold Needle. It is broken, so take it back to Gowry. He will need a moment to fix it, so just rest at a site of grace or reload the area. When you talk to him again, he will give you the repaired needle and a note called Sellia’s Secret.
How to Find the Three Beacons in Sellia
To reach Millicent, you usually have to go through Sellia, Town of Sorcery. The town is sealed by magic, and the note Gowry gave you explains how to break it. You need to light three flames atop the candle towers scattered across the rooftops.
- The first tower is on the southwest side, near the main entrance.
- The second is in the center of the town, reachable by jumping across the roofs.
- The third is on the north side, near the cliff edge.
Lighting these opens the gate at the back of the town, which leads you up the hill to the Church of the Plague. It’s just another case of beating bosses in Elden Ring.
Curing Millicent and Getting Your First Reward
At the Church of the Plague, you will find Millicent leaning against a wall, clearly in pain. When you give her the needle, she will prick herself with it and pass out. Just rest at the site of grace, talk to her until she stands up, and she will give you the Prosthesis-Wearer Heirloom. This talisman is great for early game because it gives you a flat +5 bonus to your Dexterity stat.
After this, Millicent starts her own journey. You can find her one last time in Caelid at Gowry’s Shack before she moves north. I think it is worth talking to her there because it adds a lot of context to her regaining her memories.
Moving North to Altus Plateau and the Valkyrie’s Prosthesis
Once you have progressed into the Altus Plateau, you will find Millicent standing near the Erdtree-Gazing Hill site of grace. She mentions that her arm is missing, which makes it hard for her to fight. To help her out, you need to head to the Shaded Castle, which is tucked away in a poison-filled valley to the north.
The item you are looking for is the Valkyrie’s Prosthesis. It is inside a chest in a room on the northwest side of the castle, guarded by a single Cleanrot Knight. While you are exploring this region, keep an eye out for golden seeds for upgrades because the enemies here hit like trucks and you will want those extra flask charges.
The Godskin Apostle and Windmill Village
After you give Millicent the new arm, she moves to Windmill Village in the northeast part of Altus Plateau. This place is super creepy, full of dancing villagers who will try to kill you the second you get close. At the very top of the hill, you have to fight a Godskin Apostle.
If you have been following her quest correctly, you can actually summon Millicent to help you with this fight. Once the boss is dead, rest at the new site of grace and talk to her. She is starting to realize her connection to Malenia, which is a huge lore moment. She represents Malenia’s dignity and sense of self that was lost during her battle with Radahn.
The Final Stretch in Miquella’s Haligtree
The quest then moves to the Mountaintops of the Giants at the Ancient Snow Valley Ruins site of grace. Talk to her there, then prepare for the long haul to the Haligtree. This requires getting both halves of the Haligtree Secret Medallion and solving the puzzle in Ordina, Liturgical Town.
In the Haligtree, you will find Millicent in the Prayer Room. After speaking with her, you have to head down toward the Drainage Channel. There is a massive pit of scarlet rot nearby with a Putrid Tree Spirit inside. You have to kill it to trigger the final part of the quest.
The Big Decision: Help or Betray Millicent
Once the tree spirit is dead, two summon signs will appear on the hill. This is the split in the quest where you have to make a choice.
- The Gold Sign: You help Millicent fight off her four sisters. This is the “good” ending that rewards you with the Rotten Winged Sword Insignia.
- The Red Sign: You invade Millicent and kill her yourself. This rewards you with Millicent’s Prosthesis immediately.
If you choose to help her, you can find her lying by the rot pool afterward. Do not jump on her, because the game has some weird geometry there and falling on her can actually kill her and ruin the quest. After she dies peacefully, you can loot the Unalloyed Gold Needle from her body.
Successive Attack Multipliers and Multiplicative Stacking
This is where the math gets really interesting for build enthusiasts. The talismans you get from this quest use a hidden system I like to call AccumuVal. Every hit you land builds up points, and once you hit certain thresholds, your damage jumps up in tiers.
- Winged Sword Insignia: 3%, 5%, and 10% damage boosts.
- Millicent’s Prosthesis: 4%, 6%, and 11% damage boosts, plus that +5 Dexterity.
- Rotten Winged Sword Insignia: 6%, 8%, and 13% damage boosts.
The secret to massive damage is that Millicent’s Prosthesis stacks with either version of the Winged Sword Insignia. Since the game calculates these multiplicatively, stacking the Prosthesis with the Rotten version can give you over a 25% damage boost when you are at max tier. This lasts as long as you keep attacking, but the decay starts after 1.5 seconds of inactivity.
Using Miquella’s Needle to Change Your Ending
If you helped Millicent and got the needle back, you are not done yet. You need to defeat Malenia, the Blade of Miquella. After the fight, a giant scarlet flower will appear in the arena. Using the needle on the flower turns it into Miquella’s Needle and gives you a Somber Ancient Dragon Smithing Stone. There are only 14 of these in the entire game, so it is an incredibly valuable resource.
Miquella’s Needle is the only way to reverse the Flame of Frenzy ending. You have to take it to the heart of the storm in Farum Azula, in the arena where you fight Dragonlord Placidusax. Even if you do not want to change your ending, the lore reveals that Gowry is actually a hivemind of Kindred of Rot pests who wanted Millicent to bloom into a valkyrie for his own selfish reasons. By helping her, you allow her to die with her own will intact.
